Update to client Production (Published April 2, 2026):
An updated release of 4000.12.0 (full release number:4000.12.0-rel.31+075c03f) is being applied to client Production on Thursday, April 2, 2026.

We have fixed these issues:

  • Some users experienced errors when exporting grades because the system sometimes did not start correctly. This was caused by a startup timing issue. The fix improved how the system starts so grade exports now work more reliably.

  • Instructors Cannot Grade Assignments With 2 Graders That Uses Rubrics # 92021

  • The Playback Speed menu in Video Studio was not accessible via keyboard navigation, causing an accessibility issue; this was fixed by correcting keyboard focus handling so the menu can be fully accessed using the keyboard.

Update to client Test/Stage environments (Published March 31, 2026):

An updated release of 4000.12.0 (full release number: 4000.12.0-rel.30+4e825b1) is being applied to client Test servers on Tuesday, March 31, 2026.

We have fixed these issues:

  • Some students couldn’t view their proctored test submissions after completing the exam, even when the instructor had allowed access. This issue has been fixed, and students can now view their submissions as expected.

  • When instructors edited content that included a Video Studio recording, the video sometimes disappeared after saving and did not display. This issue could affect videos in tests, assignments, discussions, journals, and forms. The problem has been fixed and edited or newly uploaded videos now save and display correctly.

  • Students Not Able to View Video Studio Video in Test Questions # 91807

  • Some institutions using a custom login page saw the biometric login button appear when it shouldn’t, causing confusion for users. This has been fixed so the biometric button now shows only on supported login.

Update to client Test/Stage environments (Published March 24, 2026):

An updated release of 4000.12.0 (full release number: 4000.12.0-rel.24+2038bd6) is being applied to client Test servers on Tuesday, March 24, 2026.

We have fixed these issues:

  • Students saw a generic “Oops! Something broke – invalid content ID” error when they tried to access a folder or subfolder that an instructor had deleted. This was fixed by updating error handling so deleted content now shows a clear, user‑friendly message (“Content is no longer available…”) with no UI break or crash.

  • When users switched courses, the app sometimes kept using old content info from the previous course, which caused an error. The fix was to load the correct content first and only then continue, so the app always uses the right course and content details.

  • Previously, editing some course content could show an “Oops” error if a linked tool was unavailable. This has been fixed so the content now handles this situation correctly without showing the error.

Update to client Test/Stage environments (Published March 17, 2026):

An updated release of 4000.12.0 (full release number: 4000.12.0-rel.24+2038bd6) is being applied to client Test servers on Tuesday, March 17, 2026.

We have fixed these issues:

  • A new, clear error modal now appears when a student tries to access a course they are not enrolled in, replacing the generic “oops” access‑denied behavior.

  • A recent change unintentionally restricted grading access in some child courses. This has been corrected, restoring normal Gradebook functionality and SIS updates while keeping unsupported actions appropriately limited.

  • AVA replies that previously did not appear in the conversation immediately have been fixed so that responses now show up right away without needing to refresh the page

  • Before, uploaded files in assignments stayed open and made the page cluttered. Now, files automatically close after upload, making submissions cleaner, easier to use, and more accessible.

  • The issue occurred when users resumed a saved draft test attempt that contained a calculated numeric question, causing the test to fail with an “Oops!/Oh no! Something broke.” error, often after refreshing the page. The fix ensures the system handles the resume process more smoothly so the attempt loads correctly without triggering errors.

  • Latex format does not display promptly in Ultra # 91591

  • Task Liveness Check Uses Incorrect Thread Name Accessor, Resulting in False Negatives( Archive Failure Emails) # 91890

  • All Gradable LTI Content are set as System Managed Incorrectly # 91983

  • LIS Integration Returning Error - String Cannot be Cast to Class [B # 91986
